Dzień dobry,

mam na imię Maciek. Urodziłem się̨ 11 grudnia 1988 r. Od początku swojego życia zmagam się̨ z chorobą. Mam wrodzoną wadę słuchu i udokumentowany umiarkowany stopień́ niepełnoprawności. Moje prawe ucho praktycznie nie funkcjonuje (80% utraty słychu), a lewe daje mi namiastkę̨ pełnego słuchu ( 20% utraty słuchu). Staram się funkcjonować́ jak zdrowy człowiek, jednak jest to dla mnie bardzo trudne.

Proszę̨ Was o pomoc w kwestii zakupu nowego aparatu słuchowego na prawe ucho oraz wsparcie finansowe w leczeniu niedrożności trąbki słuchowej lewego ucha. Deklaruję, że wszystkie niewykorzystane środki przeznaczę̨ na cele charytatywne.

Obecny czas jest dla mnie wyjątkowo trudny i newralgiczny, ponieważ w związku z pandemią straciłem pracę i moje źródło dochodu oraz utrzymania. Pracowałem jako kelner w wielogwiazdkowych hotelach w Warszawie. Z powodu utraty pracy i przewlekłego stresu, mój stan zdrowia fizycznego i psychicznego pogorszył się.

Koszt zakupu nowego aparatu to 5 990 zł. – 700 zł. dofinansowania przez NFZ. Łącznie daje to 5 290 zł. Taka kwota w najbliższym i dalszym czasie nie będzie dla mnie osiągalna do zdobycia. Kosztów leczenia niedrożności trąbki słuchowej jeszcze nie znam.

Dotychczas zawsze radziłem sobie sam i nie prosiłem innych o pomoc. Jednak nadszedł czas, kiedy zdecydowałam się zwrócić o Waszą pomoc.

Uprzejmie proszę̨ o wsparcie mnie w leczeniu oraz zakupie nowego aparatu słuchowego. W razie potrzeby mogę udostępnić dokumenty poświadczające moją chorobę.

Good morning,

My name is Maciek. I was born on December 11, 1988. I have been struggling with illness since the beginning of my life. I have a congenital hearing impairment and documented a moderate degree of disability. My right ear is practically inoperative (80% of hearing loss), and my left ear gives me a substitute for full hearing (20% of hearing loss). I try to function like a healthy person, but it is very difficult for me.

I am asking you for help in buying a new hearing aid for the right ear and financial support in the treatment of left ear trumpet obstruction. I declare that I will devote all unused funds to charity.

The time is extremely difficult and critical for me because I lost my job and my source of income and livelihood due to the pandemic. I worked as a waiter in multi-star hotels in Warsaw. Due to job loss and chronic stress, my physical and mental health deteriorated.

The cost of buying a new camera is PLN 5,990. - 700 PLN co-financing by the National Health Fund. In total, it gives 5,290 PLN. This amount will not be available for me in the near future. I do not yet know the costs of treating obstruction of the eustachian tube.

Until now I have always managed alone and did not ask others for help. However, the time has come when I decided to ask for your help.

I am kindly asking you to support me in the treatment and purchase of a new hearing aid. If necessary, I can provide documents confirming my illness. I believe that good returns, this is the principle I try to follow in my life.
